Anticoagulation & Thrombosis Management Clinic

The Anticoagulation & Thrombosis Management Clinic is where patients can get the help they need to monitor and manage the medication they are taking to prevent or manage blood clots.

Our Clinic can provide patients taking Coumadin or oral thrombin inhibitors with:

  • Scheduled blood tests for Coumadin (warfarin) dosing, monitoring and management
  • Prescription refills for Coumadin/warfarin
  • Education about possible drug and food interactions
  • Regular blood pressure checks
  • Direct communication with Primary Care and Medical Community
  • Point-of-care testing
  • Education and consultation with patients, caregivers and providers

For DVT patients, the Clinic provides:

  • Guideline-based initiation, monitoring and discontinuation of appropriate anticoagulation therapy
  • Coordination of anticoagulation therapy including ordering lab tests, adjusting anticoagulation medications, monitoring adverse reactions and compliance
  • Initiation of compression therapy to prevent Post-Thrombophlebitic Syndrome
  • Initiation of hypercoagulable work-up if appropriate
  • Education and consultation with patients, caregivers and providers

For Referring Providers

  • Patients must have a physician referral to access the services of the Anticoagulation & Thrombosis Management Center
  • Prescription assistance is available
